At Thresholds Homes and Supports, we provide a wide range of programs and services for individuals experiencing mental health or concurrent disorders.

Assertive Community Treatment

ACTT is an interdisciplinary team that provides comprehensive, community-based psychiatric treatment, rehabilitation, and support to persons with serious and persistent mental illness or concurrent issues that are complex and cause significant functional impairments.

Case Management and FACT

We offer case management services to individuals experiencing serious and persistent mental health and/or concurrent concerns, who do not require direct housing assistance from Thresholds.

Concurrent Supportive Housing

This program works to provide counselling and support for individuals who have made the choice to eliminate or change their alcohol or drug use in order to live a healthier life.

Crisis Respite Program

This program offers short-term stays, at no cost, in a residential setting for individuals 18 years of age and older who are experiencing a mental health and/or psychosocial crisis. Contact: (519) 579-0751

Crisis Stabilization Bed Program

This program offers beds for individuals residing in the Wellington County catchment area who are 18 years of age and older and are experiencing a mental health crisis. Short-term and planned crisis stabilization stays are available.

Congregate Living

Congregate living at Thresholds involves multiple people living in the same home. Thresholds offers a variety of congregate living options in Kitchener, Cambridge and Guelph ranging from homes shared by 3, 6 or 8 people.

Community Homes for Opportunity

This program is a recovery-oriented, person-centered supportive housing program for people with serious mental illness who may require 24-hour supportive care.

Extraordinary Needs Program

This program facilitates the transition of individuals with complex mental health needs from hospital to an appropriate living arrangement in the community, with individualized supports reducing over time as the person stabilizes.

Housing

We currently provide subsidized housing in Waterloo Region and Guelph-Wellington, through agency-owned properties and partnerships with private and non-profit sector landlords.

Housing Support Services

All individuals housed in properties owned or overseen by Thresholds receive a range of ongoing support services from a Housing Support Coordinator that respond to their individual needs.

Partners in Active Living

The Partners in Active Living includes individuals who are supported by the organization, who meet monthly to socialize and plan special events and activities.
