Here 24/7 Referral Process
This referral process applies to the following programs and services:
- Housing Support Services
- Case Management
- Concurrent Supportive Housing (CSH)
All referrals for these programs and services are handled through Here 24/7.
Crisis Respite Program
No referral is necessary. For more information or to complete a telephone screening, please call Crisis Respite directly at (519) 579-0751
Assertive Community Treatment Team
An ACT Referral form for Waterloo Region needs to be completed through Here 24/7. New referral applications are reviewed by a Community Intake Group. You can contact them at 1-844-437-3247 (1-844-HERE247) to complete eligibility screening and be placed on the waitlist for assessment.
Each referral is assessed to confirm that ACTT is the appropriate level of support required. Consumers are referred to the Assertive Community Treatment Team from other less intensive teams or services when they require more frequent contact that only an ACT Team can provide.
Extraordinary Needs Program
Referrals to the Extraordinary Needs Program are completed by hospitals or community agencies. A referral package is available by contacting the referral organization or contacting Thresholds.
Crisis Stabilization Bed Program
Referrals are accepted from anyone. For further information, referrals, or to apply to the Crisis Stabilization Bed Program, please contact Thresholds.
