Community Homes for Opportunity

Community Homes for Opportunity (CHO) is a recovery-oriented, person-centered supportive housing program for people with serious mental illness who may require 24-hour supportive care. The program is intended to assist tenants by providing appropriate housing and support services to achieve and maintain housing stability that is safe and affordable.

Program Features

The program provides access to both in-home and community support services designed to improve and/or stabilize people’s physical and mental health, foster independence, and promote participation and integration into the community.

The services are flexible, reflecting the unique and changing needs of the individuals served. Following a recovery model, the CHO home environment empowers clients to be active participants in their own planning and ensures they have the opportunity to make individual choices.

The program offers opportunities for tenants to enhance the quality of their daily living, focus on their personal growth and development, and improve life skills through participation in a variety of activities and programs.
