A Note from Holt Sivak, Executive Director

It is my pleasure to welcome you to our recently updated website.  We continually strive to provide you with the most up-to-date information about Thresholds Homes and Supports and our activities.

Our latest publication, the 2014-2015 Annual Report is now available for your viewing (please Click here or visit the News and Publications section).  The Annual Report reviews our activities of the past year and gives a brief look at what our plans are for the coming year.

We are especially proud of two new initiatives.  The Extraordinary Needs Program supports individuals with very complex mental health needs, while the Homemaker (HNSA Homemaker and Nurses Services Act) Program enables us to provide homemaking and life skills support services to individuals with mental health issues.

Thank you for your interest in Thresholds.  Please do not hesitate to contact me if you have any questions or comments.
