Housing Support

From shared residences with 24/7 staff support to independent units with monthly check-ins, Thresholds Homes and Supports is proud to offer a full spectrum of supportive and affordable housing options to meet clients’ individual needs and goals. 

While programs have varying eligibility criteria, clients for all programs must:

  • be 18 years of age or older,
  • voluntarily agree to enter the program;
  • be able to live safely in community with available supports,
  • live in the area where the program is being provided or have a plan to move there.

HSS | Housing Support Services

Housing and Support Services (HSS) offer affordable housing with personalized supports to meet client goals – ranging from community participation to supportive counselling, to nursing support.

Clients accessing HSS may live in independent units throughout our service areas or live in one of our homes with shared amenities and staffing.

Individuals who:

  • meet general criteria listed at the top of this page;
  • have a primary diagnosis of one or more of the following serious and persistent mental health illnesses for at least two years:
    • Schizophrenia spectrum or other psychotic disorders
    • Bipolar or related disorders
    • Anxiety or Depressive disorders
  • meet at least 3 of the 5 categories of functional disability, as a result of mental illness not primarily relating an acquired brain injury or developmental disability:

    • requires support with instrumental activities of daily living such as managing finances, managing transportation, shopping and meal preparation, house cleaning and home maintenance, managing communication and managing medications;
    • Is unemployed, is employed in a sheltered setting or supportive work situation, or has markedly limited skills and a poor work history
    • Safety concerns related to self or others, or exhibits inappropriate social behavior which results in intervention by the mental and/or judicial system
    • Has difficulty in establishing or maintaining a personal social support system/ limitations or moderate impairment in social functioning
    • Requires public financial assistance from out-of-hospital maintenance and may be unable to procure such assistance without help
  • are willing and motivated to engage with support worker on a weekly to monthly basis in goal-oriented support.

ENP | Extraordinary Needs Program

The Extraordinary Needs Program (ENP) provides a fully supported home environment to facilitate the transition of individuals with complex mental health and other needs from hospital to a long-term shared residence.

This not only ensures limited hospital spaces are opened for those who need them, but individuals receive appropriate care in a home setting.

THS’s dedicated ENP homes are located in Kitchener or Guelph.

Individuals who:

  • meet general criteria listed at the top of this page
  • live with a mental illness and/or addiction;
  • have complex issues, including but not limited to: acquired brain injury, dual diagnosis, concurrent disorder, difficult to support behaviour, fluctuating needs, and complex medical needs;
  • are currently in hospital and designated Alternate Level of Care (ALC) and/or has had 6+ hospital admissions and/or the individual displays a pattern of behavior which repeatedly puts themselves or others at risk, which is a barrier to housing stability. This would be demonstrated by at least two of the following:
    • Persistent homelessness and/or repeated loss of housing;
    • Need for intensive housing supports;
    • Repeated hospital admissions, EMS and/or emergency department visits;
    • Involvement with the criminal justice system;
    • Significant support efforts from multiple community agencies; or significant disengagement from community services;
  • have primary barrier(s) to housing stability that are not related to a diagnosis of dementia or a physical disability or an acute medical condition;
  • have explored all options for discharge from hospital or appropriate housing and support in the community, including consultation with service resolution (where applicable) and there is no feasible community housing plan.

Transitional Housing

With wraparound care at its centre, the Transitional Housing program empowers previously unhoused individuals to work towards long-term housing in the community in a safe, shared residence with 24/7 staff support.

Our only transitional housing site currently is located in Guelph.

Individuals who:

  • meet general criteria listed at the top of this page;
  • are chronically homeless;
  • are registered on the by-name data;
  • are willing and motivated to participate in programming requirements;
  • and are able to take care of their own activities of daily living (hygiene, etc.) independently;
  • have the capacity to live independently after this period of recovery.

Referrals for this program are made through Wellington County’s Housing Stability Department’s Housing Stability Working Group.

CHO | Community Homes for Opportunities

Community Homes for Opportunities (CHO) provides administrative, advocacy, and recreational support for individuals experiencing mental health issues and living in select privately owned and Ministry funded affordable shared homes. 

Thresholds Homes and Supports (THS) manages funding and oversight for these homes, serving as a client advocate to maintain standards and improve accommodations and care, including the quality of meals provided or housing related concerns. THS further supports residents by providing recreational opportunities. 

Individuals who:

  • meet general criteria listed at the top of this page;
  • have diagnosed or diagnosable severe and persistent mental illness,
  • are compliant with prescribed medication and follow-up;
  • do not require complex care normally associated with Long-Term Care facilities;
  • do not have a history of behaviour that poses a serious fire hazard.
