Community Treatment Teams

Thresholds Homes and Supports’ Flexible Assertive Community Treatment Team (FACTT) and Assertive Community Treatment Team (ACTT) are multidisciplinary teams including social work, housing support, substance use treatment, occupational therapy, nursing, and psychiatry.  

Clients live with severe and persistent mental health diagnoses, such as schizophrenia and bipolar, and may move between teams as needs change. Both teams share the same eligibility criteria and referral process:

Individuals who:

  • are 18 years of age or older,
  • voluntarily agree to enter the program;
  • live in the Waterloo Region or Guelph-Wellington or have a plan to move there;
  • have a primary psychotic disorder, bipolar disorder, or severe treatment resistant depression/anxiety
  • have high service needs, such as:
    • high use of  hospital, crisis, or emergency services
    • involvement with the criminal justice system
  • have demonstrated intensive support needs, such as:
    • difficulty adhering to medication
    • housing issues
    • severe, persistent functional impairment
    • difficulty engaging with supports
  • have not had success with lower intensity services

In the absence of a clear Axis I diagnosis, individuals must also:

  • have psychosis and/or major mood disorder symptoms that are a significant/primary barrier to functioning in community 
  • have had a previous positive response to psychiatric medications that are required to maintain stability in the community
  • have a historic pattern of psychiatric symptoms showing chronicity and acuity
  • show a high need on a priority/risk screener (completed by staff upon referral)

FACTT| Flexible Assertive Community Treatment Team

Serving clients one to three times per week, the FACTT team provides flexible, person-centred support to individuals who typically manage their daily living and medications independently, but benefit from consistent connection to maintain stability. The team walks alongside clients, offering guidance, encouragement, and practical support to help them stay well in their community.

ACTT | Assertive Community Treatment Team

With daily supports, ACTT provides direct intensive case management including medication supports to the most vulnerable clients within the community, through a harm reduction approach regardless of their housing status.
