Supportive Housing

Our Housing and Support Services provide individuals with comprehensive longer-term support. Support is based on the individual’s recovery goals which may include domains such as; employment, education, volunteering, recreation or becoming more engaged and integrated in the community. Individuals housed in properties owned or overseen by Thresholds Homes and Supports receive a range of ongoing support services from a Housing Support Coordinator that respond to their individual needs. The supports included in the program include coordinating community supports, crisis support, supportive counselling, practical supports for daily living (ADL), nursing support, and recreational connections. All of Thresholds’ supports are rooted in the strengths, goals and direction of the individuals we support.

 

Housing options include congregate living with seven (7) other individuals, smaller shared living homes, and independent subsidized apartments. Individuals receive ongoing support services that range from 24/7 on-site support to monthly contact.

 

Support Coordination

In addition to these directly owned and managed units, Thresholds provides support within a number of non-agency managed accommodations. These include boarding homes, domiciliary hostels, room rentals and market rent units. In these non-agency managed accommodations we coordinate with other community program to provide appropriate support without duplication of services.

 

Eligibility Criteria

  • 18 years or older

  • Must live in the Region of Waterloo or County of Wellington or have a plan to move to either area

  • Diagnosed or diagnosable severe and persistent mental health issue

  • Willing and motivated to engage with support worker on a weekly to monthly basis in goal-oriented support (monthly basis is the minimum meeting requirement)
