Thresholds Homes and Supports is a recovery-oriented, community-integrated agency that focuses on improved quality of life and enhanced independence for people experiencing mental health issues by providing access to affordable housing and flexible, individualized support.

Our Role in the Community

Crossing a threshold is the beginning.

At Thresholds Homes and Supports, we believe that a stable, permanent home is the first step towards mental wellness. We offer a range of mental health services, including ACTT, FACT, supportive housing, crisis respite, case management, and more.  We assist individuals in the communities of Kitchener-Waterloo, Cambridge, and Guelph-Wellington.

292 Sheldon Avenue

In response to the urgent need for supportive housing, Thresholds Homes and Supports has started construction on a three storey, 25-unit deeply affordable housing project on our property at 292 Sheldon Avenue N. in Kitchener.

The 25 self-contained, low barrier, energy efficient units will provide the opportunity for residents to receive support through stabilization and recovery.  Our goal through this project is to demonstrate a housing model that highlights the benefits of integrated and inclusive communities while providing forward thinking support for those at risk.

292 Sheldon is made possible through funding from both Waterloo Region and CMHC, our partners in addressing the immediate need for supportive housing.

Crisis Respite Program

The Crisis Respite program offers short-term stays, at no cost, to individuals who are experiencing a mental health concern or other crisis. This voluntary, non-medical, and supportive program is offered in Kitchener. Contact: (519) 579-0751

Supportive Housing

Our organization offers a number of supportive housing options through our own properties and various community partnerships. Our housing options range from shared furnished homes to single apartment units.

Assertive Community Treatment Team (ACTT)

ACTT is an interdisciplinary team that provides intensive case management to individuals with severe and persistent mental health or concurrent issues. Our approach focuses on the support, rehabilitation, and treatment of individuals.

Case Management

Thresholds Homes and Supports offers mental health case management services to individuals experiencing serious and persistent mental health and/or concurrent concerns, who do not require direct housing assistance.
